View your medical records
Medical records hold information about you. They are sometimes called health records.
Your GP medical record includes:
- past conditions
- allergies
- medication
- test results
- appointment notes and letters
You will have different records for other NHS services you’ve used, including hospitals, dentists and opticians. To access these records, you’ll need to contact the relevant service.
Using the NHS app or website
The NHS app is a quick way to access NHS services. Using the NHS app means you do not need to enter your personal details each time you contact the surgery. You can read more about the app on the NHS website.
You can view your GP medical record online using your NHS account.
You will need the NHS app or your NHS account login details.
As of November 2023, all patients can see their detailed coded records. If you would like to see your records prior to this date, you will need to submit our full medical records access form and is subject to appropriate verification checks and safeguards.

Visit the Practice in person and hand to reception your completed application form. Please inform the receptionist whether you’ve read the information leaflet.
Please Note that we also have the application form and information leaflet at reception if you are unable to download, complete and print it.
Show your Identification – two forms of ID. one photo ID such as Passport, drivers licence, freedom pass etc and also one form of ID with your home address such as recent utility bill, bank statement etc.
Once you have completed the above steps, the practice will process your registration and email you a Pin Document which you need in order to gain full access to your digital medical records.
Once you have registered your account, you will have full access to all the online services you requested
