Rapid Health Triage Guide
How to make an appointment
You can book an appointment online through our Rapid Health system. To help make your experience as smooth as possible, please read and follow the steps below.
Step 1 – Click Access Rapid Health Triage at the bottom of the page
Step 2 – Click Adult medical request if you have a NEW medical symptom or problem
OR
Click Questions and admin if you need one of the following:
- To ask the doctors a question about your medication, symptoms that have not resolved, treatment, or to book a follow‑up – click Ask a question
- Request a letter or report
- Further information you may need regarding your referral
- Fit‑note request
- Test‑result requests (check the NHS app first)
- Repeat‑prescription request
- Travel clinic request
- Update personal details (This can be done via the NHS app or on our website : link: Update your details « Hampton Medical Centre
- Register for electronic prescribing
OR
Click Self‑help information for trusted NHS advice on your symptoms or conditions.
Step 3 – Provide your information: fill in your name, date of birth, postcode, phone number, and email address.
Step 4 – Answer questions carefully to book the correct appointment for your issue or request.
Based on your responses, you may not be able to book an appointment immediately, but your request will be seen by a doctor by the end of the next working day and they will either call or email you with an appointment.
Please avoid sending numerous requests about the same issue.
Please note that you will be contacted by email or phone.

We will NOT be using the online system for the following appointments, which can still be booked by contacting Reception:
- Routine practice‑nurse appointments
- Annual reviews with a nurse (e.g.: Diabetes, Asthma, COPD, Blood pressure)
- Home visits
- Palliative‑care patients
- Pharmacist appointments
If you do not have access to the internet, you can still call the surgery and one of our Patient Services Advisors will guide you through the triage form.
Carers and housebound patients – If you look after somebody, are looked after by a carer, or are housebound, please tell our reception team. It is important that we have this information on your medical records so we can provide appropriate care and support.

You can also access Rapid Health using the NHS login & NHS App
NHS login
If you access Rapid Health using your NHS login details, the identity verification services are
managed by NHS England.
NHS England is the controller for any personal information you provided to NHS England to
get an NHS login account and verify your identity and uses that personal information solely
for that single purpose.
For this personal information, our role is a “data processor” only and
we must act under the instructions provided by NHS England (as the “data controller”) when
verifying your identity.

NHS App
You can access Rapid Health on the NHS App using your NHS login details.
If you sign in using NHS login, we will ask your permission to share your NHS login
information with our service. This allows us to fill in some personal details for you, such as
your name, date of birth and contact details.
We will not use your NHS login information for any other purposes. You can only share your
NHS login information if you have proved your identity to NHS login.
You can choose not to share your NHS login information with Rapid Health but you will need
to enter your information yourself whilst using the service.
